Morris County Distinguished Military Service Medal Application

The Morris County Board of County Commissioners wishes to commemorate all veterans who served honorably in the United States military by presenting the Morris County Distinguished Military Service Medal to eligible Morris County residents.Eligibility

To be eligible for a medal, you must:

  1. Demonstrate you were honorably discharged from the United States Armed Services
  2. Be a current or former resident of Morris County

To apply for a medal, you must provide:

  1. A copy of your DD-214 record (All Personal Identifiable Information contained in a DD-214 will remain strictly confidential whether or not you are approved for a medal.)
  2. A copy of Active Duty card, if currently in the reserves

If you believe you are eligible, please fill out this form to apply for a Morris County Distinguished Military Service Medal.

Ceremony & Public Notice of Service Medal

Morris County holds two or more public ceremonies each year, at which veterans who are approved to receive Morris County Distinguished Military Service Medals are presented with their honors. Participation in a ceremony, however, is not required to receive a medal.

At the end of this form, you will be asked if you want to participate in one of the annual ceremonies. If you want to participate in a public ceremony, answer “yes.” Our Veterans Services Office will be in touch with you about next steps. If you do not want to participate, answer “no.” Your medal will be mailed to you.

NOTICE: Morris County Distinguished Military Service Medals are bestowed at the discretion of the Morris County Board of County Commissioners, with the intent of honoring area veterans, living or deceased, for their service to our nation. The Commissioners reserve the right to share publicly the name, address and terms of military service of any veteran approved for the honor, regardless of whether an applicant chooses not to participate in a ceremony. By applying for a Morris County Distinguished Military Service Medal, and upon notification that you have been approved to receive a medal, you are agreeing to the public disclosure of your name, address and terms of military service in connection with receipt of the honorary medal. The involvement of Veteran Services Officers within the Morris County Department of Human Services in processing applications does not constitute an expectation of privacy or confidentiality regarding an application for a Morris County Distinguished Military Service Medal.
